This course will provide therapists with assessment and intervention strategies to promote optimal functional cognitive recovery and outcomes in the ICU and acute care setting. Emerging evidence suggests that early cognitive intervention in acute care significantly reduces delirium incidence and duration as well as the adverse effects of surviving critical illness. This course will provide knowledge and tools for assessing, monitoring or improving key cognitive abilities in critically ill and acute care patients. A framework for integrating cognitive activities into the medical ICU and acute care environment will be discussed within the context of a team approach. Case examples including those with COVID-19 will be presented, and participants will be asked to apply knowledge and methods to case scenarios throughout the presentation.
- Describe the effects of delirium and critical illness on outcomes
- Choose delirium assessments, mini cognitive screenings, and cognitive functional assessments suitable for the acute care environment.
- Implement strategies to engage families of critically ill patients in treatment that supports cognitive functioning.
- Implement an early cognitive rehabilitation program for critically ill adults using a framework for ICU cognitive interventions and activities
- Apply how to select and tailor a wide range of cognitive functional activities in acute care, so they are at an optimal level of challenge for the client
- Design treatments to simultaneously address both physical and cognitive goals within the acute care environment.
- Develop strategies for team coordination and collaboration in addressing cognitive symptoms and for optimal discharge planning
- Discuss emerging evidence that supports early cognitive intervention in critically ill patients
Joan Toglia, Ph.D, OTR/L, FAOTA is a Professor of Occupational Therapy and Dean of the School of Health & Natural Sciences at Mercy College in Dobbs Ferry, New York. She is also an Adjunct Clinical Professor of Cognitive Science in Rehabilitation Medicine at Weill Cornell Medical College. Dr. Toglia has over 30 years of experience in occupational therapy and is internationally recognized as a leader in the rehabilitation of persons with cognitive deficits. In addition to her degree in Occupational Therapy, Dr. Toglia completed advanced graduate and doctoral work in Educational Psychology with specializations in cognition & learning as well as measurement & evaluation.
Dr. Toglia has contributed extensively to the field of cognitive perceptual rehabilitation as a clinician, author, educator, and researcher. She has presented over 300 workshops and lectures around the world on cognitive rehabilitation, focusing on theory, assessment methods and intervention strategies to support and optimize cognitive function across different ages and diagnostic groups. Her publications and research include metacognitive strategy intervention, assessment tools and the relationship between cognitive impairments and occupational performance.
Chelsea Steinberg, OTR/L, C/NDT is the Outpatient Occupational Therapy Clinical Specialist at New York-Presbyterian/Weill Cornell Medical Center in New York, New York and an adjunct teaching associate of the Occupational Therapy graduate program at Mercy College in Dobbs Ferry, New York. She graduated from the University of New Hampshire in 2012 and completed the first AOTA accredited residency program in acute care at the University of Chicago Medical Center in 2014. She has worked for 7 years in adult acute, inpatient and outpatient neurological rehabilitation settings and has advanced knowledge and clinical skill sets in rehabilitation of persons with cognitive deficits. Her recent experience includes working on a COVID-19 recovery unit.
Ms. Steinberg has collaborated with Dr. Toglia on a number of cognitive research projects and studies at New York-Presbyterian Weill Cornell. She has presented several lectures and workshops focusing on assessment methods and intervention strategies to support and optimize cognitive function, including three national workshops at ACRM and the AOTA Annual Conference. Her publications and research have focused on application, feasibility and treatment fidelity of the Multicontext approach to cognitive rehabilitation.
