Improving Outcomes for the Medically Complex Patient: Acute Care, Home, Rehab, SNF, OPD

Course Description:

This course will provide you with the most up-to-date, evidenced-based techniques for assessment and treatment of several major chronic diseases as well as teach you techniques to motivate your patients to follow through with their exercise programs. Multiple morbidities/chronic diseases impact the medical complexity and hence require modifications to prognosis, interventions and functional outcomes. Standardized tests and measures to use during documentation of functional assessments will be identified to demonstrate your effectiveness to payers and referral sources. Populations and co-morbidities will include:

  • Chronic Metabolic Disease: Obesity, Diabetes
  • Musculoskeletal Disease: OA and Osteoporosis
  • Cardiac Disease: CAD and Hypertension
  • Chronic Pulmonary Disease: Asthma and COPD
  • Neurological Disease: Parkinsons

Course Objectives:

  • Advance therapy skills to meet the demands of healthcare industry changes that impact therapy chronic disease management.
  • Integrate standardized tools into the assessment of patients with chronic disease.
  • Develop evidenced based, individualized chronic disease treatment plans based on an understanding of the pathophysiology, complications and prevention of secondary sequelae.
  • Apply the use of behavior modification techniques to promote patient chronic disease self management.
